  5. What is the My Account - Beneficiary Information page? faq id: 1650

    The My Account – Beneficiary Information page is where you may enter and update the information of your kit's beneficiary. The beneficiary is the person who will be granted control of your myFTDNA account, your DNA test results, and any stored DNA sample in the event of your death. After they contact us, they will gain complete control of the kit and will be authorized to order additional tests with any DNA that may remain in stock.

    • Name - This is the beneficiary's name.
    • Phone - This is the beneficiary's phone number.
    • E-mail - This is the beneficiary's e-mail address.

    To update this information, fill out the form and click the Save button. Once you have saved the information, a link to a Printable Form will be available. Note that because the form uses the information on your My Account - Contact Information page, you should update that page before proceeding. If you would like a hard copy of this to include in your will or with your other legal documents, click the Printable Form link. This will take you to the form. Verify your information, print it, and have it notarized.

    The My Account - Contact Information page is where you may enter and update your personal address and your e-mail address.

    • Prefix - (Optional) Enter a prefix for your name such as Miss or Mr.
    • First Name - This is your first or given name.
    • Middle Name - (Optional) This is your middle name, if you have one.
    • Last Name - This is your surname or family name.
    • Suffix - (Optional) If you have one, this is the suffix for your surname, i.e., Sr., Jr., etc.
    • Address - This is your current mailing address, i.e., your street name and house and apartment number.
    • City - This is the city or town in which you currently live.
    • State - If you live in the US, this is the state in which you live.
    • Zip - If you live in the US, this is your postal zip code.

    • Project Administrator Settings - For each project to which you belong, you may choose to allow the administrators of that project to change your settings and personal information.
    • Project Administrators with Full Access - When you have granted a project administrator full access to your myFTDNA account, their name is shown in this table. You may revoke access by changing your password.
